Your first visit to our Rancho Cucamonga practice lays out the foundation for trust, commitment, and lasting relationships with our patients. It’s the beginning of our journey together in maintaining the beauty and health of your smile.
During the first visit, we’ll review your medical history, take a series of new x-rays, and complete a thorough examination of your teeth, gums, and jaw. This will help us get a better understanding of your oral health and expectations.
Your first visit is also a good time to ask any questions you may have about our practice and services. To save time, you can print out and complete our downloadable patient forms below before arriving at Renaissance Dental Care for your appointment.
New Patient Forms
To save time during your first visit, we ask that you bring your insurance card or policy number, along with a list of current medications. Filling out these forms ahead of time will allow Dr. Stockdale and our staff to provide you with the absolute best care.

Children should have their first dental appointment no later than their first birthday. This is around the age when children’s primary teeth, also referred to as baby teeth, start developing and become visible along the gumline. A child’s first visit is focused on teaching them proper oral hygiene habits and explaining the importance of maintaining good dental care.
Yes, it’s important to update your x-rays once a year to look for signs of infection or decay, and keep track of your oral health. If you’re a new patient, we’ll require you to take a series of new x-rays to record any progress made with our team at Renaissance Dental. Our digital x-rays are completely safe and don’t cause any long-term complications.
During a professional dental cleaning, our hygienists will check for signs of tooth decay or gum disease, and remove any plaque and tartar build-up. We’ll also thoroughly clean and polish your teeth, giving you a fresh and healthy smile. It’s important to schedule a dental cleaning at least twice a year to maintain your winning smile and good oral health.
If you can’t print out the forms, don’t worry! We’ll have the same forms available to you at our Rancho Cucamonga office. We recommend coming in 10 to 15 minutes before your appointment to fill out the forms in a timely manner and help us provide you with the best possible care. We also ask that you bring your insurance information and a list of current medications.
